29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: a.) Terminal Ready Mix located at 524 Colorado Ave. Lorain, OH 44052; On or about June 8, 2016, the employer did not ensure an amputation injury was reported to the local OSHA office within 24 hours. An amputation injury occurred on May 13, 2016, and was not reported until June 6, 2016.
