(a) In the manufacturing plant, 50 Oxford Ave., Dudley, MA 01571, on 6/7/2016 an inpatient hospitalization resulting from a workplace injury occurring on 5/26/2016 had not been reported to OSHA within 24 hours of the incident occurring.
