29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) In the trailer production facility: On or about May 18, 2016, and at times prior thereto, the employer failed to ensure that powered industrial truck operators were trained and competent to operate forklifts safely, exposing employees to hazards not limited to struck-by and crushed-by. Big Tex Trailer Manufacturing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number 314096074, citation number 1, item number 1 and was affirmed as a final order on April 3, 2012, with respect to a workplace located at 502 South Midway Rd., Cordele, GA 31015.
