On or about May 16, 2016 and times prior thereto, in the Beef Production area, employees were exposed to a caught-in hazard when operating a Hollymatic Tenderizer 1200 without a point of operation guard. b. On or about May 16, 2016 and times prior thereto, in the Patty Making area, employees were exposed to a caught-in hazard when operating a Hollymatic Super 54 Patty Making machine without a point of operation guard.
