29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite, First Floor, Lot 25 Forest Oaks Trail, North Huntington, PA 15642: Observed on or about April 8, 2016, each employee on a walking/working surface was not protected from falling through an unguarded stairwell opening. The stairwell opening on the first floor measured approximately 8' wide and 7' 4" long and was not equipped with a mid rail. Employees engaged in framing activities on the first floor of the home were exposed to a fall hazard of approximately 11 feet to the unfinished (gravel) basement floor below. b) Jobsite, Second Floor, Lot 25 Forest Oaks Trail, North Huntington, PA 15642: Observed on or about April 8, 2016, each employee on a walking/working surface was not protected from falling through an unguarded stairwell opening. The stairwell opening on the second floor measured approximately 4' 6" wide and 7' 4" long and was not equipped with guard rails. Employees engaged in framing activities on the second floor of the home were exposed to fall hazards of approximately 21 feet to the unfinished (gravel) basement floor below. c) Jobsite, second floor window openings, Lot 25 Forest Oaks Trail, North Huntington, Pennsylvania 15642: Observed on or about April 08, 2016, each employee walking/working on, at or above wall openings where the outside bottom edge of the wall opening was greater than 6 feet or more above lower levels were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. The inside bottom edge of the wall openings measured less than 39 inches (1.0 m) above the walking/working surface at several windows on the second floor. Employees who were working on the second floor of the house were exposed to fall hazards of up to approximately 25 feet to the ground below. Pierce Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 912011, citation number 1, item number 1 and was affirmed as a final order on 7/12/2013, with respect to a workplace located at #8 Unit Bayberry Road, Canonsburg, PA.
