29 CFR 1904.29(a): An OSHA 300 form or equivalent form was not used for recordable injuries and illnesses: (a) Ark Woodworks Inc, at 878 Baseline Place, Brighton, CO: On and preceding 4/4/16, the employer did not ensure that an OSHA 300 form or equivalent form was used for recordable injuries and illnesses. An employee sustained a recordable injury in 2015 which resulted in approximately one day of lost time and seven days of job restrictions. An OSHA 300 form or its equivalent was not used for the recordable injury.
