29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: Express Packaging client warehouse located at 1155 Harvester Rd., West Chicago, IL, 60185 - On or about March 15, 2016, the employer's issued certifications for powered industrial truck operators included the identity of a person that did not perform training and evaluation of the operator's performance. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
