29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #401 Mazak SL60/120 (model no. SLANT TURN 60N, serial no. 137409) which exposed employees to entanglement hazards. (b) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on (Machine #402 Mazak Integrex (model no. INTE50YR-2500, serial no. 140316) which exposed employees to entanglement hazards. (c) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #403 Doosan Daewoo Puma 400C (model no. Puma 400LB, serial no. P35L1943) which exposed employees to entanglement hazards. (d) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #404 Mazak (model no. Slant Turn 60NR, serial no. 181093) which exposed employees to entanglement hazards. (e) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #406 Mazak Nexus 350 (model no. INTE50YR-25000, serial no. 140316) which exposed employees to entanglement hazards. (f) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #405 Mazak SL60/120 (model no. SLANT TURN 60N, serial no. 1908698) which exposed employees to entanglement hazards. (g) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #409 Mazak (model no. QTN-450M-II, serial no. 193693) which exposed employees to entanglement hazards. (h) On or about March 15, 2016, the safety interlocks were bypassed on Machine #411 Mazak 4500-1 (model no. CYBERTECH TURN 4500M, serial no. 233306) which exposed employees to entanglement hazards. (i) On or about March 15, 2016, in the lathe department in the East Building, the safety interlocks were bypassed on Machine #414 Mazak 4500-2 (model no. CYBERTECH TURN 4500M, serial no. 238697) which exposed employees to entanglement hazards. (j) On or about March 15, 2016, in the East Building, the Kuraki CNC Horizontal Boring/Milling Machine (model no. KBM-11X) was not guarded properly in that employees were not protected from moving parts of the machine.
