Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to being struck by a bi-directional machine: (a) DE-INK MILL/YARD AREA -- EMPLOYEE(S) EXPOSED TO HAZARD OF BEING STRUCK BY A FORD NEW HOLLAND TRACTOR, MODEL 445C TRACTOR DURING OPERATION OF FURNISHING PULPER(S) AND MOVING PAPER MATERIAL IN YARD; THE OPERABLE HORN WHICH HAD BEEN INSTALLED AS A MEANS OF ABATEMENT WAS MISSING AND THE MECHANISM TO ACTIVATE THE REVERSE ALARM WAS NOT OPERATIONAL. The American Tissue Mills of MA, Inc. was previously cited for violation of this Occupational Safety and Health Standard, or it's equivalent standard, Section 5(a)(1), which was contained in OSHA Inspection No. 102768389, Citation No. 01, Item No. 01, issued on 10/11/94.
