29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Crimping machines on AN, UMA1, UMA2, LFA, and JFA lines; On or about January 26, 2016 and at time prior thereto, the employer exposed employees to potential hazardous energies (including electrical and pneumatic) that could result in crush by and amputation injuries in that employees were allowed to perform servicing and/or maintenance on the crimpers without those employees being provided with machine specific lockout tagout procedures to prevent the accidental start-up of the machine or the release of hazardous energy. b) Air Leak Testers on AN, UMA1, UMA2, LFA, and JFA lines; On or about January 26, 2016 and at time prior thereto, the employer exposed employees to potential hazardous energies (including electrical, pneumatic, and compressed helium gas) that could result in caught-in and struck by injuries in that employees were allowed to perform servicing and/or maintenance on the air leak testers without those employees being provided with machine specific lockout tagout procedures to prevent the accidental start-up of the machine or the release of hazardous energy. PYONGSAN AMERICA, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.147(c)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315760033, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON December 13, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 760 WEST VETERAN'S BLVD, AUBURN AL 36830.
