29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 1/20/2016, Blending Area: Lock out procedures were not developed nor utilized while employees were performing cleaning operations on the Con 5 line, Machine #15. Abatement documentation is required for this item. The Raymond Hadley Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i) which was contained in OSHA inspection number 962561, citation number 1, item number 5a, and was affirmed as a final order on 4/25/2104, with respect to a workplace located at 89 Tompkins Street, Spencer, NY.
