29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a/ Inside of the Mandrel Line, the ingoing nip-points belonging to the Boston Matthews Caterpillar Puller, m/n unknown, s/n CWL110DC9556, were inadequately guarded. One operator suffered the loss of an arm on or about 12/23/15. b/ Inside of the Mandrel Line, the in-going nip-points belonging to the Farris Universal Processing Machine, m/n unknown, s/n C650VT5764, were inadequately guarded. Inside of the Inspection Line, the rotating parts and ingoing nip points belonging to the following machines were unguarded: a/ Line #1, Coiling Machine and the Let-Off Wheel. b/ Line #2, Coiling Machine and the Let-Off Wheel. c/ Line #3, Coiling Machine and the Let-Off Wheel. d/ Line #4, Coiling Machine and the Let-Off Wheel. e/ Line #5, Coiling Machine and the Let-Off Wheel. f/ Line #6, Coiling Machine and the Let-Off Wheel. Inside of the Extrusion Line, specifically the north side, the ingoing nip-points belonging to the following machines were inadequately guarded: a/ Line #2, Ferris Universal Processing, unknown m/n or s/n. b/ Line #2, Knittner Machine, unknown m/n or s/n. c/ Line #6, unknown make, model, or serial number. d/ Line #8, unknown make, model, or serial number. Inside of the Extrusion Line, specifically the south side, the ingoing nip-points belonging to the following machines were inadequately guarded: a/ Line #1 Boston Matthews, m/n C110DCMP, s/n C550 1091. The Parker Hannifin Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 329139, citation number 1, item number 1, and was affirmed as a final order on 02/08/13, with respect to a workplace located at 1200 Ely Street, Kennett, MO 63857.
