29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) 443 -447 Marlene Way Leola, Pennsylvania - Employees worked from a platform supported on the forks of a Manitou MVT1203L rough terrain forklift, serial number 400818. The platform was 16 feet 10 in above the lower level and the employees did not use an approved method of fall protection, on or about November 16, 2015. b) 443 -447 Marlene Way Leola, Pennsylvania - Employees worked from a platform supported on the forks of a Manitou MVT1203L rough terrain forklift, serial number 410007. The platform was 16 feet 10 in above the lower level and the employees did not use an approved method of fall protection, on or about November 16, 2015. c) 443 -447 Marlene Way Leola, Pennsylvania - Employees worked from a platform which spanned approximately twelve feet between two supported platforms on the forks of Manitou MVT1203L rough terrain forklifts. The platform was approximately 16 feet 10 in above the lower level and the employees did not use an approved method of fall protection, on or about November 16, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
