Text For Citation: 02 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while being lifted on a forklift truck with an unapproved personnel lift platform: a) Warehouse area, employees were using forklift trucks with wood pallets as a personal lift platform for access up to the second and third tier levels in the tire storage racks. b) Warehouse area, employees were climbing the frames of the storage racks to access the second and third levels of the storage racks to pull orders and stock tires. While ultimate responsibility for correcting the hazard rests with the employer, given his/her superior knowledge of the work place, feasible and acceptable methods to correct this hazard, among others are: (1) Follow guidelines listed in the American Society of Mechanical Engineers (ASME) B56.1- 2000, Safety Standard for Powered Industrial Trucks, section 4.17 - Elevating Personnel - with a High Lift Truck designed to elevate personnel, and section 7.36 - Platforms - Elevating - provide the restraining means such as railing, chains, cable, body belt(s) with lanyard(s), or deceleration devices, that are in place and properly used. (2) Follow guidelines listed in the American National Standards Institute (ANSI) A64.1, Requirements for Fixed Industrial Stairs, sections 3.1 - Where Fixed Stairs Are Required - for access to elevations which requires regular travel between levels. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations(s).
