OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and/or struck by hazards of hydraulic automotive lifts: a. 450 N. Broad Street, Elizabeth, NJ - Automotive lift, position 5 The employer did not repair automotive lifts as prescribed in ANSI ANSI/ALI ALOIM-2008. Employees engaged in automotive repair activities worked on vehicles, elevated on hydraulic automotive lifts that exhibited mechanical defect and were not tagged and removed from service. Automotive lifts used by employees did not maintain pressure when elevated with a vehicle on the lift. Safety Leg locks did not automatically engage and required employees to walk under an elevated vehicle to engage the locks. Violation observed on or about February 22, 2016 b. 450 N. Broad Street, Elizabeth, NJ - Automotive lift, position 2, 3, 4, 5, and 6 The employer did not repair automotive lifts as prescribed in ANSI ANSI/ALI ALOIM-2008. Employees engaged in automotive repair activities worked on vehicles, elevated on hydraulic automotive lifts that exhibited physical defect and were not tagged and removed from service. Automotive lifts had broken/damaged arm extensions that did not have stops to prevent the extension from becoming disconnected from the lift arms. Violation observed on or about February 22, 2016 c. 450 N. Broad Street, Elizabeth, NJ - Automotive lift, position 1, 2, 3, 4, 5, 6, and 7 The employer did not inspect hydraulic lifts as prescribed in ANSI ANSI/ALI ALOIM-2009. Employees engaged in automotive repair activities worked on elevated vehicles from hydraulic lifts that were not inspected per the American National Standard outlining the safety requirements for proper vehicle lift operation, inspection and maintenance requires that a qualified lift inspector inspect every lift at least annually. Violation observed on or about November 3, 2015, February 12, 2016, and February 22, 2016 d. 450 N. Broad Street, Elizabeth, NJ - Automotive lift, position 1, 2, 3, 4, 5, 6, and 7 The employer did not ensure automotive lifts had product identification tags. Hydraulic automotive lifts used by employees to perform automotive repairs did not have product identification tags that included, catalog number or model number, serial number, date of manufacture, or rated load capacity.
