Executive Order 12196 Section 1-201(a) of the Occupational Safety %% and Health Programs for Federal Employees: The agency head did not furnish employees places and conditions of employment that were free from recognized hazards that were causing or were likely to cause death or serious physical harm: a) Employees were exposed to the hazard of suffocation, burns, or other serious bodily harm or death where air traffic controllers serving at the Teterboro NJ Airport were not aware of a site specific fire emergence evacuation plan. Training and information about the details of the plan were not provided. Actual evcuation exercises were not conducted. The handling and use of fire extinguishers during a fire emergency were not explained and demonstrated. The elevator alarm system was inoperative and the fire service operating instructions were not posted in the elevator cab on or about 12/2/92.
