29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipment grounding connection were used: (a) At the temporary desk for the Blencoe office, the continuity to ground was interrupted between a 2-prong 125 volt wall receptacle used to provide power to a 3-prong relocatable power tap (RPT) when an AC three-prong to two-prong plug adapter was used. This condition exposed workers to a fire and electrical hazard. The US Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(3)(iii), which was contained in OSHA inspection number 1004473, citation number 1, item number 3 and was affirmed as a final order on 02/17/2015, with respect to a workplace located at 100 East 3rd Ave, Ellensburg, WA 98926.
