29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) and (c). (a) 1857 Pebble Court, Harrisburg Pennsylvania - An employee entered an excavation that was not provided with a sloping and benching system or a supported system, shield system, or other protective systems. The employee became entrapped when one of the side walls of the trench collapsed and buried him to his waist, on or about September 30, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
