29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: On or about 09/18/2015, an employee working for S. Bollinger & Associates, LLC located at 5462 Hillsboro Hematite Road, De Soto, MO, received a work related struck-by incident which resulted in the employee passing on 09/19/2015. The employer failed to contact OSHA within the required 8 hour period.
