29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: a) Extrusion department; On or about September 1, 2015 and at times prior; the employer exposed his employees to a 12.10 feet fall hazard in that employees were allowed to walk in a platform with an unguarded portion in front of the access ladder when performing tasks such as verify the HVAC system. Genpak, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(2), which was contained in OSHA inspection number 315194811 citation number 1 item number 1 and was affirmed as a final order on June 2, 2011, with respect to a workplace located at 7621 Bill Joseph Parkway, Hope Hull, Al 36043
