29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Employees were exposed to pinch point and thermal hazards created by the sealer bar movement on the two ARPAC TS-33 and four Great Lakes TS-37 machines in various lines located throughout the production floor. (b) Employees were exposed to rotating shafts in the four Great Lakes TS-37 machines as the film door guarding the machine was not interlocked and frequently open when running in various lines located throughout the production floor. (c) Employees were exposed to caught-in and pinch point hazards created by the machine in-feed movement from the autoloader located in line 10. (d) Employees were exposed to ingoing nip points created by rotating conveyor belts and pulleys on the Doboy Cup Nester located in the IPA line. (e) Employees were exposed to the pinch point created by the Doboy Mustang IV Crimper head as it rotated cutting and sealing plastic bags in the IPA line. (f) Employees were exposed to the pinch point created by the Ameripak Crimper head as it rotated cutting and sealing plastic bags in the IPA line. (g) Employees were exposed to ingoing nip points created by vertical conveyors on the Hayseen line.
