Text For Citation: 02 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that two employees were exposed to the hazard of fire/explosion: a.Site - An automatic methane detection system had not been installed, on or about 1/3/96. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to install an automated methane detection system whereby if the levels of combustible gases reached 25of the LEL, all electrical equipment in the enclosure would be shut down. b.Site - The relief valve on the compressor gas receiver was not vented to a safe location, on or about 1/3/96. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to comply with ASME Boiler and Pressure Vessel Code Section VIII, paragraph UG-135. c.Site - The relief valve on the compressor head was not vented to a safe location, on or about 1/3/96. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to comply with ASME Boiler and Pressure Vessel Code Section VIII, paragraph UG-135. d.Site - The spacer vent on the Gardner-Denver compressor was not vented to a safe location, on or about 1/3/96. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to pipe the valve to a safe area. e.Site - A hand gate valve adjacent to the relief valve on the head was not vented to a safe location, on or about 1/3/96. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement would be to install an automated methane detection system whereby if the levels of combustible gases reached 25of the LEL, all electrical equipment in the enclosure would be shut down.
