29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made, aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard and permanent aisles and passageways were not appropriately marked: a) K-Plant, behind #1, 2, 3 and 6 presses, 6th and Center Streets, Tarentum, Pennsylvania 15084: Observed on or about July 31, 2015, sufficient safe clearances were not provided for the movement of forklifts and hoppers behind the presses. Employees were required to move forklifts and hoppers within feet of electrical equipment including motor control centers (480 volt, 3 phase) which were labeled as HRC level 0 and level 4 arc flash and shock hazards. Forklift operators were exposed to the hazards of electric shock and electrocution. b) Warehouse, between one and two rows, in front of shower room, 6th and Center Streets, Tarentum, Pennsylvania 15084: Observed on or about July 31, 2015, a pothole existed in the warehouse floor between one and two rows. Employees were exposed to exposed to injuries while operating forklifts over the pothole. c) Warehouse, between eight and nine rows, 6th and Center Streets, Tarentum, Pennsylvania 15084: Observed on or about August 28, 2015, a pothole existed in the warehouse floor between rows eight and nine. Employees were exposed to exposed to injuries while operating forklifts over the pothole. RESCO Products, Inc. was previously cited for this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.176(a), which was contained OSHA Inspection Number 966608, Citation #1, Item #2, and was affirmed as a final order on August 19, 2014 with respect to a jobsite located at 6th and Center Streets, Tarentum, Pennsylvania 15084.
