Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a recognized hazard of electrical shock, burns, and/or electrocution. a) M/V Galaxy: Employee in the #1 freezer hold had occasion to use the orange, shop built extension cord that had the polarity reversed. This condition subjected to employees to the possibility of using defective tools and becoming a source to ground, resulting in shock, burns, or electrocution. Among other methods, one feasible and acceptable abatement method to correct this hazard is to rewire this cord to meet National Electrical Code standards or purchase new factory built extension cords, and inspect and test all cords on a regular basis. Dutch Harbor Seafoods, Ltd. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard which was contained in OSHA inspection number 107100190, Citation Number 1, Item Number 2, issued on 03/22/89.
