29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident On or about June 4, 2015, at a construction site located at 6550 South Hwy. 360, Grand Prairie, TX 75052, at 18:21 a fatality occurred. The employer did not report the fatality to OSHA within eight hours of the incident. OSHA was notified on June 23, 2015 of the fatality by the victim's next of kin.
