29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board: On or about May 1, 2015, the employer failed to ensure employees performing guide changes on the Sutton 3-2 Roll Straightener were protected against a fall of 3 feet into a slew way hole/pit. The pit is filled with sludge, coolant and sharp broken steel shards.
