On or about April 30, 2015, at the facility of FWD/Seagrave Fire Apparatus, located at 105 E. 12th Street Clintonville, WI 54929, employees were exposed to an unguarded Cincinnati press brake that was not adequately guarded to prevent employees from getting any part of their body in the point of operation. b.
