29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about 4/3/15, an employee was exposed to cave-in hazards in an inadequately sloped trench without receiving any form of training from the employer on the recognition and avoidance of the unsafe conditions along with the regulations applicable to Subpart P of the standard. L&H Gyr Excavating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2) which was contained in OSHA inspection number 107318, citation number 1, item number 1 and was affirmed as a final order on 3/21/12, with respect to a workplace located at Martin Ave. Fond Du Lac, WI 54937.
