29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the forks and that the forklift was not moved horizontally while the platform was occupied. On or about 31 March 2015 and at times prior thereto, two workers were unloading materials from a job-site made, wooden trash box supported by but not attached to the forks of a rough terrain forklift. The trash box was elevated to upper levels, including a height of up to approximately 36 feet of the apartment building under construction, exposing employees to a fall hazard.
