29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Shipping and Receiving - On, or about, March 11, 2015, the company failed to guard the hopper on the J-Nate Extruder, serial number JC00202, model number J180G to prevent any body part of an employee from entering the danger zone(s) during operation. The Plainville Plating Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 212(a)(1), which was contained in OSHA inspection number 3143987210, citation number 1, item number 22 and was affirmed as a final order on August 31, 2010 with respect to a workplace located at 21 Forrestville Avenue, Plainville, Connecticut 06062.
