29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 5, 2015, employees were exposed to struck-by, caught-in, and amputation hazards when working on or walking near the Perfect Pleat lines #5, #10, and #3. a. The guard on the tractor tribar on line #5 is not secured and is opened during setup in order to monitor alignment. b. The guard on the tractor tribar on line #3 is not secured and is opened during setup in order to monitor alignment. c. The saw is inadequately guarded on line #5. d. The saw is inadequately guarded on line #3. e. The saw is inadequately guarded on line #10 and the guard was missing a bolt. f. Pleat pack inserter is inadequately guarded on line #5. g. The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 931623, citation number 1, item number 2 and was affirmed as a final order on 02/19/2014, with respect to a workplace located at 2624 Weaver Way, Atlanta, GA.
