29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Sawmill - On or about March 6, 2015 employee was inside dust pit below head rig and carriage to clean sawdust off walls and lubricate unguarded dust chain conveyor. The unguarded conveyor inside the pit was running and the head saw and carriage were not locked out. (b) Sawmill - On or about March 6, 2015 employees were changing the blades on front chipper hog and the power was not tagged out. (c) Sawmill - On or about March 6, employee was observed standing on conveyor performing cleanup next to edger with unguarded belt and pulleys and the conveyor and edger were not lockout out. (d) Sawmill - On or about March 6, 2015 employees were observed cleaning around saws, unguarded belts and pulleys of motor drives and the equipment was not locked out.
