29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): The employer does not guard the point of operation on machines whose operation exposes employees to injury. a. This violation occurred on or about March 03, 2015, and at times prior thereto, on the north side of the facility where employees were exposed to struck-by and caught-by hazards while using an Iowa Precision brand slear machine without a point of operation guard. b. This violation occurred on or about March 03, 2015, and at times prior thereto, in the Layout Department where employees were exposed to struck-by and caught-by hazards while using a double ended combination machine without a point of operation guard. c. This violation occurred on or about March 03, 2015, and at times prior thereto, in the Layout Department where employees were exposed to struck-by and caught-by hazards while using a R-5020H roll bending machine without a point of operation guard. d. This violation occurred on or about March 03, 2015, and at times prior thereto, in the DP Department where employees were exposed to a struck-by hazard while using a damper press without a point of operation guard. e. M & M Manufacturing Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 212(a)(3)(ii), which was contained in OSHA inspection number 316045236, citation number 1 item number 1 and was affirmed as a final order on June 06, 2012, with a final abatement date of June 20, 2012, with respect to a workplace located at 4001 Mark IV Parkway, Fort Worth, TX 76106. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all the point of operations on all machines are guarded.
