29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer but did not involve death or days away from work, the employer did not record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column. (a) Human Resources Department, 2015 OSHA Form 300: The establishment incorrectly recorded a job transfer or restriction case as an other recordable case -- 1904.7(b)(4), for an injury that occurred on February 18, 2015. The injury was sustained when a Mental Health Assistant was punched between the neck and shoulder by a patient and was placed on restriction for 6 days but the injury was recorded as an "Other Recordable" on or about May 22, 2015. (b) Human Resources Department 2015 OSHA Form 300: The establishment incorrectly recorded a job transfer or restriction case as an other recordable case -- 1904.7(b)(4), for an injury which occurred on February 19, 2015. The injury was sustained when a Registered Nurse was punched and pushed to the floor by an agitated patient and placed on restriction for 3 days, on or about May 22, 2015. Bergen Regional Medical Center, LP was previously cited for this occupational safety and health standard or its equivalent standard 1904.29(b)(1), which was contained in OSHA inspection number 978372, citation number 2, item number 1 and was affirmed as a final order on 7/22/2014 with respect to the workplace located at 230 East Ridgewood Avenue, Paramus, NJ 07652.
