Section 5 (a) (1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of electrocution, electrical explosion, and burns resulting from the failure to adequately monitor faults to ground in eletrical distibution systems, and to quickly find and repair faults to ground as the occur. One feasible and acceptable abatement methods, is to set up phase monitoring on all systems requiring monitoring; to frequently check the phase monitor for faults to ground; and then to quickly and re pair the faults: Dept 09, South Side Block House-One phase of the 3 phase power supply to #9 crane and a ship on the water was intermittently going to ground.
