29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. (a) 3715 E Farm Road 94, Springfield, Missouri: On or about January 20, 2015, the employer failed to provide a copy of the first report of injury to the OSHA Compliance Safety and Health Officer for an accident that occurred on January 19, 2015. Copies of the first report of injury was not provided to the OSHA Compliance Officer to take with the officer within four business hours of the request.
