29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by 29 CFR 1910.178(l): a) This violation was observed on or about January 20, 2015, January 21, 2015, and January 23, 2015, throughout the work place; where employees operated forklift trucks without having first completed training consisting of formal instruction, practical training, and an evaluation of the operator's performance in the workplace.
