29 CFR 1910.1450(e)(3)(vii): The employer's Chemical Hygiene Plan did not include the designation of personnel responsible for implementation of the Chemical Hygiene Plan, including the assignment of a Chemical Hygiene Officer; and when appropriate, establishment of a chemical hygiene committee: (a) Location - Establishment On or about 1/7/15, the employer's Chemical Hygiene Plan did not designate the personnel responsible for implementation of the Chemical Hygiene Plan and the personnel assigned as the Chemical Hygiene Officer. Polycarbon Industries, Inc. DBA PCI Synthesis was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.1450(e)(3)(vii), which was contained in OSHA inspection number 927156, citation number 1, item number 5a and was affirmed as a final order on 11/2/2013, with respect to a workplace located at 88 Jackson Rd., Bldg. 649, Devens, MA 01434.`
