Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of severe burn injuries created by explosions and fire in the foundry at the melt deck and furnaces. a)On or about 12-30-96, Foundry, Melt Deck, at Furnace #3, an explosion occurred, following charging of the furnace that had not received proper pre- heating and inspection, causing numerous fires and burn injuries to two employees.. b)On or about 01-09-97, Foundry, Melt Deck, at Furnace #2, an explosion occurred, following charging of furnace that had not received proper pre- heating and inspection, causing numerous fires. Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: 1.Inspect each load of scrap metal as it arrives at the facility to ensure there are no enclosed casings, fittings, or boxes. 2.Inspect each load as it enters the facility to ensure that there is no snow or ice within the load. 3.Follow manufacturer's specific instruction for operation and use of the Vanetta Pre-Heat Oven, including correct loading of the pan, prevent overloading, correct time of pre-heat, uniform temperature of the load. 4.Train employees in the correct procedures to operate the Vanetta Pre- Heat Oven and operations at the melt deck. 5.Provide a warning system for the Melt Shop when the furnaces are to be charged. 6.Ensure that employees are in protected areas when the furnaces are to be charged.
