Text For Citation: 02 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a. Multiple injuries from fall hazards while working on Column IC2 which was setting on 2 bolts with leveling nuts in the column web, with the column base being approximately 1 3/4 inch from the concrete pier, and out of plumb approximately 1.77 ft. with the existing pipe rack pier where the 40 ft. beam #11B6 would be connected; was not adequately supported by the use of shims between the column base plate and the concrete pier or by the use of guy lines, or by installing beam #13B4 as a bent prior to connecting beam #11B6 to Column IC2, on or about July 15, 1993.
