29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. On or about October 28, 2014; at the Central Rock (main office), the OSHA logs of recordable occupational injuries and illnesses required to be kept under part 1904, were not provided within four (4) business hours upon request for calendar years 2011, 2012, 2013.
