Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) Employees working in the area of the High Line Pipe bridge adjacent to the areas monitored by the Number 2 Control Room were exposed to the hazards from failure of overhead pipe conduit support structures. These hazards include fire, explosion, exposure to toxic gases and physical hazards from collapse of piping, conduit and other support structures. The following conditions contributed to the existence of these hazards: 1) Abandoned piping has been allowed to deteriorate to the point that support structure is not adequate. EXAMPLE: A 1 1/2 inch insulated pipe running between pipe tower #1 and #7 is supported at only 4 locations. The pipe passes over roadways. It is located on the east side of the pipe bridge. EXAMPLE: An 8 inch pipe on the west side of the pipe bridge travels between towers 2 and 13. The pipe is supported only by other piping and support struts. EXAMPLE: A 4 inch insulated pipe on the west side of the pipe bridge, spanning the distance between towers 3 and 9 is supported only by other piping and support struts. EXAMPLE: A conduit bank on the east side of the pipe bridge between towers 1 and 10 has support hangers disconnecte for much of the length at Tower #1.
