29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) SL-6 Fascia Line - The guarding on the roll forming machine was inadequate exposing employees to the hazards of in-running nip points. The guards did not prevent the inadvertent entry of hands into the machine from the feed end, and near the discharge end where the protective plastic film is attached to the fascia. These guards are not secured from opening during the operating cycle of the machine or interlocked to deactivate the machine if a guard would be opened during operation. (b) SL-6 Fascia Line - The guard (hood) over the pattern embossing machine was inadequate exposing employees to the hazard of in-running nip points between the embossing rollers. The hood was not secured or interlocked to prevent it from being opened during machine operation. (c) SD-1 Double 5 Siding Line - The guarding on the roll forming machine was inadequate exposing employees to the hazards of in-running nip points. The guards did not prevent the inadvertent entry of hands into the machine from the feed end, and near the discharge end where the protective plastic film is attached to the fascia. These guards are not secured from opening during the operating cycle of the machine or interlocked to deactivate the machine if a guard would be opened during operation. (d) SL-6 Fascia Line - The Massman Automation Wrap Around Case Packer (Autopacker) was in operation with the safety interlocks on the various access doors disabled allowing employees to gain access to the equipment and be caught-in and/or struck by the moving machinery. On or about October 17, 2014, an employee operating this equipment received serious injuries while retrieving a tool from inside the protective enclosure and the safety interlocks did not stop the machine's hazardous motion. The machine was observed operating in this condition on October 22, 2014. (e) SL-6 Fascia Line - The Massman Automation Palletizer was in operation with the light curtain safety device deactivated on the end of the unit where the packaged product is stacked on pallets that are removed for shipping. The light curtain has not been operational for several years. The palletizer was observed to be in operation in this condition on October 22, 2014 and December 3, 2014. The Rollex Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 98541, citation number 1, item number 2 and was affirmed as a final order on 11/15/2011, with respect to a workplace located at 800 Chase Ave., Elk Grove Village, IL 60007. The Rollex Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 441178, citation number 1, item number 1 and was affirmed as a final order on 08/15/2012, with respect to a workplace located at 800 Chase Ave., Elk Grove Village, IL 60007.
