29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge on how to inspect, put on, and remove, use, and check the seals of the respirator: a- Hovensa Refinery St. Croix, Penex and Alkylation Control Rooms - Operators could not effectively don their Scott Ska-Pak airline respirators which must be used during emergency events. Conditions noted on or about 07/06/09, 07/07/09 and 07/08/09.
