29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) On or about January 30, 2018, a receptacle box located between the maintenance and fabrication shops had an opening not effectively closed where a knockout plug was missing, exposing employees to an electrical shock hazard. b) On or about January 30, 2018, breaker box located in the center of the fabrication shop had openings not effectively closed where multiple knockout plugs were missing, exposing employees to an electrical shock hazard. c) On or about March 22, 2018, two receptacle boxes which had knock out holes that were not effectively closed were being used along the conveyor on the bending line, exposing employees to an electrical shock hazard.
