29 CFR 1910.141(a)(3)(i): All places of employment were not kept clean to the extent that the nature of work allowed. b- Hess Port Reading Refinery, Gas Con and FCCU - Debris such as but not limited to wooden boards/planks, rags and insulation was found throughout the units on or about process equipment to include vessels, exchangers, pumps and towers. Condition noted on or about 12/18/08 and 01/05/09.
