29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed. On or about October 08, 2014, and at times prior thereto, the employer did not ensure that the power supply conductor holes on the power control box for job made saw #3 were effectively closed, exposing employees to the hazards of electrical shock.
