29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: In the following instances each circuit breaker and/or box was not labeled as to the equipment it controlled, exposing employees to the hazards associated with delays in turning off equipment and/or turning off the wrong equipment. a) On or about January 30, 2018, in the fabrication shop near the maintenance shops, the breaker boxes were not labeled to identify each breaker's purpose. The breakers control the lights, receptacles and welders in the shop areas. b) On or about February 6, 2018, at the entrance of the fabrication shop, the breaker box was not labeled to identify each breaker's purpose. The breakers control the lights, receptacles and welders in the shop area. c) On or about March 22, 2018, in the paint storage building, the breaker box was not labeled to identify each breaker's purpose. The breakers control the lights and receptacles. d) On or about March 22, 2018, in the assembly building, the breaker box was not labeled to identify each breaker's purpose. The breakers control the lights and receptacles, iron workers and plasma cutter. e) On or about February 6, 2018, in the fabrication shop, employees were working around an area where there was an energized 240 volt conductor lying on the floor with the outer sheath pulled back and the copper end unprotected, exposing employees to the hazard of an electrical shock. f) On or about March 29, 2018, in the fabrication shop, on the 110 volt 5 ton winch used to lift tanks lids, the electrical control panel covers were missing , leaving unprotected conductors open to contact, exposing employees to the hazards of electrical shock.
