29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a. PM #9, Drive Room 2- On or about November 27, 2019; an employee, deenergizing the 2400 volt fused disconnect for the #2 Jones Refiner, was not protected from electric shock and arc flash hazards where properly tested and inspected electrically insulating rubber gloves were not provided; personal protective equipment for the lower body, appropriate for the task, was not provided; properly tested and inspected fuse handling equipment was not provided; and high voltage testing equipment was not used to verify an absence of voltage. b.
