29 CFR 1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: On or about October 08, 2014, and at times prior thereto, electrical equipment with damaged parts was not taken out of service or repaired, exposing employees to the hazard of electrical shock. a. In the spinner department, the 480 volt conduits running along the edge of spinners #10 and #11 were bent, broken and corroded. b. In the End Plate Department on mold oven #1, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box. c. In the End Plate Department on mold oven #2, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box. d. In the Metal Storage Area, the power control box on/off switches were broken for compressor #2 and recessed in the control box requiring that the employees put their fingers into the control box. e. In the Saw Department on Saw #1, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box. f. In the Saw Department on Saw #2, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box. g. In the Machine Shop on lathe #24, the power control handle was broken and missing. h. In the Machine Shop on lathe #40, the power control handle was broken and missing.
